What is Geriatric Care Management?

Emerald Care’s Geriatric Care Management services take a holistic, client-centered approach to helping older adults and families experience aging in a safe, supportive, autonomous, and satisfying way. Care Management helps structure a seamless experience, improve quality of care, promote client safety and independence, and relieve anxiety for the client and family.

What does a Geriatric Care Manager do?

They take a series of steps to help solve problems of older adults facing health care challenges. They also help the older adults and their families implement and monitor these solutions. A Care Manager can also provide preventative services with the goal of helping families anticipate possible needs in the future so that everyone feels supported and better prepared. They forge a positive path forward for you and your loved ones.

How do I know if I need Geriatric Care Management Services?

Are you helping care for an older loved one and it’s not going as well as you hoped? This is not at all uncommon. Reasons for geriatric care management are varied, but are often due to:

  • Lack of time to give because of other responsibilities;
  • Geographical distance between you and your loved one;
  • Challenging family dynamics;
  • Unfamiliarity or lack of understanding with complex medical or legal issues; and/or...
  • You’re not really sure what to do, what needs to be done, or how to proceed forward?

Five Star Geriatric Care Management

The Emerald Care’s Five Star Geriatric Care Management service helps solve the difficult problems of older adults and their families such as:
  • Health concerns – physical and psychosocial
  • Functional concerns – ability to bath, dress, toilet, etc.
  • Medication compliance
  • Safety in the home
  • Caregiver burnout
  • Family dysfunction that affects the care of an older person
Emerald Care’s professional Geriatric Care Managers will:
  • Pinpoint the problems of the older adult (via a complete Geriatric assessment),
  • Come up with needed solutions (via a Care Plan),
  • Implement the solutions, and
  • Monitor the solutions.
These solutions will ultimately maintain the older person’s independence, provide peace of mind to their loved ones, and support their quality of life.

Geriatric Assessment

The Geriatric Assessment can be a stand-alone service and is also included in the other Emerald Care Geriatric Care Management services. The Geriatric Assessment is a detailed written report that evaluates a problem that an older person is having which results in a care plan. The care plan focuses on solutions to the older person’s problem(s) and proposed implementations.

Some of the problems may include:
  • Can they remain safely at home?
  • What type of care and equipment do they need to remain safely at home?
  • How do you find the right in-home care and equipment?
  • How to safely discharge home after a hospitalization?

Medication Management Services

These services assure older adults, families, and physicians that a client’s prescribed medications are managed and taken correctly.
  • Emerald Care’s licensed Registered Nurse (RN) will set-up prescribed medicines as ordered by the client’s physicians,
  • Work towards the goal of achieving safe self-administration or family administration of medications,
  • Work closely with the client’s physician and pharmacist to help the older client achieve the best outcomes from the course of treatment.

Home from the Hospital Services

This service solves the problems of older adults who must enter a hospital and be discharged home.
  • Emerald Care’s professional Geriatric Care Managers will assess, coordinate, guide, educate, and advocate for the client, family, and family caregiver from pre-admission through admission; hospitalization, discharge, and transition back home.
  • Preventing readmission is a major benefit of this service through monitoring clients after discharge, supporting their caregivers and family, monitoring medication and arranging home health services for rehabilitation.
  • These services can also be used when older adults transition to and from both skilled nursing and rehabilitation facilities.

End of Life Services

These services can be provided to clients and their families facing a disease process that may lead to terminal illness. Emerald Care professional Geriatric Care Managers, with their foundation of years of experience, can provide information for making health care decisions, preparing for death, achieving closure, supporting emotional well-being, helping advocate for the best care, enhancing self-efficacy while gaining self-confidence in the ability to care for family members, assisting with monitoring the paid caregivers, and working closely with the hospice interdisciplinary team.
Our care managers will assess, coordinate, guide, educate, and advocate for the client and family through the five stages of death and dying.

Concierge Companion Services

Emerald Care’s Concierge Companion services provide extra care and monitoring for an older person who lives in an assisted living or skilled nursing facility.
Emerald Care’s professional Geriatric Care Managers will monitor and enhance both the quality of care and quality of life of the client by ensuring that the client is receiving the right care and companionship – along with any other support the client needs – by finding an ideal companion.
A care plan will be prepared by the Geriatric Care Manager based on both a Geriatric and a Quality of Life assessment. The companion will provide the care which is monitored by the care manager. The care manager can also attend facility care conferences and medical appointments to advocate for the client
