The Department of Oregon Human Services (ODHS) registers Long-Term Care Referral Agencies and Agents who assist with referrals to Long-Term Care Communities. Emerald Care Support Services (Emerald Care) is the Registered Agency, and Nancy Bond, our owner and administrator is the Registered Agent.
A Long-Term Care Referral Agent provides referral services to a wide variety of Long-Term Care Communities. These communities include Assisted Living, Residential Care, and Nursing Facilities, Memory Care Communities, Continuing Care Retirement Communities and Adult Foster Homes. These communities provide care and assistance for those with memory loss, dementia, Alzheimer’s, Parkinson’s, those who have been affected by a stroke, have mobility issues, and many other medical/health issues.
As a Registered Nurse, Ms. Bond has over ten years of experience assisting older adults experience aging in a safe, supportive, and autonomous way by taking a holistic, client-centered approach. As a Certified Care Manager, Ms. Bond is also a problem solver who has provided solutions to clients in many different situations. These solutions have ultimately improved the quality of life of our clients and helped reduce family stress.
Ms. Bond promises to help you and your loved ones save time and frustration by assisting you with locating and choosing a Long-Term Care Community that best matches your needs. Whether you’ve planned ahead or a sentinel event has occurred which changes your situation thereby requiring immediate placement, Ms. Bond will negotiate and advocate on your behalf to ensure the transition is as smooth and expedient as possible.
Our process begins with a consultation over the phone to gather information about your needs to determine the best course of action. Our placement process is individually customized based on functional care needs, financial resources, and lifestyle and location preferences.
A home visit is scheduled to collect the rest of the information needed to help you and your loved ones make an informed decision. We don’t make decisions for you – instead we educate, facilitate, guide, support, and consult to assist you in making the final placement decision. We then research suitable community options, review two years of compliance history for each of the chosen communities, tour the selected community with you, assist with admission to the chosen community, and finally, follow up with you after you’ve settled into your new home.
There is no cost for our referral services – instead, the referral fees are paid by the community you choose. We have working relationships with the Communities we believe are the “best of the best” and therefore, our referrals are welcomed by them. These relationships create a much better experience for everyone involved – especially you!
